Your free MRI review is for informational purposes ONLY. The information provided is not a form of diagnosis or recommendation of treatment. A diagnosis and treatment recommendations can only be made after you have been examined by one of the medical providers at Saratoga Spine.

2. After submitting the form, print out the form details
3. Mail the copy of your form entry and your test results to Saratoga Spine's central office at:

Instructions to upload the MRI/CT and report:
1. Put CD of MRI in your computer
2. Open DICOM folder on CD
3. Select folder(s) in the DICOM folder
4. Right click and choose: send to, compressed (zipped) folder
5. You will get a message cannot create compressed folder. Do you want it placed on the desktop instead? Say YES
6. Right click on the name of the file on your desktop. Rename the file with your first initial and last name.
7. Go back to Free MRI review and choose the file you just compressed to your desktop & upload
8. Authorize Saratoga Spine to review your test results and medical information
9. Submit
Whenever possible, please upload the MRI/CT images AND the report.

You will see a screen acknowledging your information has been uploaded.

Our office will call you as soon as we review your information.
